Primary care network (PCN)

We are a member practice of MWEB PCN, Primary Care Network. This currently comprises of Moulton Surgery, Woodview Medical Centre, Kingsthorpe Medical Centre, Denton Village Surgery and Wootton Medical Centre.

In total MWEB provide services for over 30,000 patients.

Since the NHS was created in 1948, the population has grown, and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.

To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

PCNs are led by clinical directors who may be a GP, general practice nurse, clinical pharmacist or other clinical profession working in general practice.

MWEB PCN are a progressive thinking and supporting team who strive to provide safe, cost effective, evidence-based care to all our patients.

Within our PCN workforce, at Kingsthorpe Medical Centre, we have a Pharmacist, a Pharmacy Technician, a Social Prescriber, two First Contact Physios, a Paramedic who looks after our Care Home, a Safeguarding Co-ordinator, Access to our Ageing Well Team and a Care Co-ordinator, who supports us with the administration of the PCN Workforce. This is addition to our Practice Team.
